I'm in the process of filling out the paperwork for my wife's permit. There is one thing new that I noticed since I did mine. They (Nassau) now require a NYS DMV "lifetime driving record" rather than just the abstract. In order to get that, you have to send an MV-15 form to Albany and wait for them to send you back the "lifetime abstract". I cannot understand the need for this other than just to make it more difficult for people to apply. The instructions for that are here: https://dmv.ny.gov/get-my-own-lifetime-driving-record
Agree with everyone else, there is no reason to use or pay for a service to do this for you. The instructions / forms are straightforward. Disclose EVERYTHING that the form asks and you should be fine. For anything you check "Yes" to, include a notarized statement in as much detail as possible ESPECIALLY if you have EVER been arrested. I've uploaded a template you can use for any statements. Feel free to PM or post here with any questions.

That's just to slow people down - now we have to wait for the DMV to receive the form and check, hope they don't lose it, then process the form, print the abstract, bag it (shove into envelope), and chuck it into the mail back to us.

I agree. The lifetime driving record really is a reach and annoying. The application clearly asks only about traffic violations in the past 5 years; the abstract that you can get online from MyDMV covers that. Why they need the lifetime record is beyond me.
